1 The aortomitral curtain is the fibrous structure that connects the aortic and mitral valve annuli, making the 2 valves interdependent.2 Surgical aortic valve

The anterior leaflet has a semi-circular shape and attaches to two fifths of the annular circumference. There is continuity between the anterior leaflet of the mitral valve and the left and non-coronary cusp of the neighboring aortic valve, referred to as the aortic-mitral curtain. The aortic (AV) and mitral (MV) valves are coupled via a common fibrous continuity known as the aortic–mitral curtain or intervalvular fibrosa . As a result, the physiological function of either of these two valves involves the complementary function of the other [ 2 ]. Operative findings showed an extracardiac aneurysm of the interleaflet triangle above the aortic-mitral curtain. Because there was no sign of active inflammation, the orifice was closed with an autologous pericardial patch, and the aortic valve was replaced with a mechanical valve.

On either end of the aortic-mitral curtain is a triangular junction of fibrous tissue known as the right and left fibrous trigones (Figure 4 & 5). Cardiac involvement in reactive arthritis is well-recognized, and usually results in aortic regurgitation, proximal aortitis, and conduction system abnormalities. A common structure, the subaortic curtain, 14–17 simultaneously supports 2 of the aortic cusps (left coronary and noncoronary) and the anterior leaflet of the mitral valve.

However, in patients with preserved aortic annulus, reconstruction of the AMC is possible using a simple technique with limited incisions. Case presentation A handmade bovine three-portion pericardial patch was used to Five-hundred and fifty-seven (557) subjects received isolated aortic replacement, 176 received isolated mitral replacement, and 29 received replacement of both the aortic and mitral valves. The cumulative follow-up for all subjects was 773.51 patient-years with a mean follow-up of 1.02 patient-years (s.d. = 0.71 patient-years, range 0 – 3.10 patient-years).

The aortic valve is opened during systole, the driving force for it to open is the difference in pressure between the contracting left ventricle of the heart and the aorta. along with valvular lesions, the aorto-mitral curtain (AMC, junction between base of anterior mitral leaflet and aortic root) thickness is increased on transthoracic echocardiography. In other words, at the MA-Ao junction, the left atrial wall does not join the left ventricular wall but is attached to the aorta. The aortic-mitral curtain is the fibrous tissue between the anterior mitral valve leaflet, the left and non-coronary cusps of the aortic valve, and the left and right trigone.
